Hypertensive retinopathy

by Dr Adrian Koay

A 46 year-old man was admitted to the medical ward for severe hypertension with a blood pressure of 230/140 mmHg. He also complained of blurring of vision in his left eye for 3 days. His visual acuity was 6/6 in the right eye and 6/12 in the left eye. He was referred to us because the physician noted bilateral optic disc oedema. Dilated fundoscopy revealed bilateral type IV hypertensive retinopathy.

Severe hypertensive retinopathy of the right eye with mild disc swelling, generalized arterial

narrowing, flame haemorrhages, cotton wool spots and hard exudates.

The left fundus shows Grade IV hypertensive retinopathy similar to the right eye.
