Operculum and retinal hole

by Professor Chua Chung Nen and Dr Adrian Koay

A 52 year-old man presented with sudden onset right floater. The visual acuity measured 6/9. Slit-lamp examination showed posterior vitreous detachment and an operculum. Fundoscopy with a superfield 90D lens revealed a retinal hole with subretinal haemorrhage at the superotemporal retina. Argon laser was performed to prevent retinal detachment (150 X 200um X 200mW X0.1sec).

The risk of retinal detachment is less with retinal hole than retinal tear.

Slit-lamp view of posterior vitreous detachment and operculum.

Relation between the operculum (red arrow) and the retinal hole (blue error) at the peripheral superotemporal retina through the view of a 90D superfield lens.

Another view of the above through slit-lamp fundoscopy.
