HIV retinopathy

A 63 year-old developed shortness of breath and the chest X-ray showed changes suggested of Pneumocystic Carinii pneumonitis (PCP). The HIV test was positive. During treatment, he complained of blurring of vision. Examination revealed bilateral multiple cotton wool spots typical of HIV retinopathy. Unlike CMV retinitis, the whitish lesions are multiple and do not enlarge in size. These cotton wool spots occur primarily in the posterior pole in the peripapillary region.
