Terson's syndrome

Photo by Professor Chua Chung Nen

This middle-aged man was referred by the neurosurgeon because of complaints of seeing dark patches in both eyes. He was recently treated for subarachnoid haemorrhage and an operation was carried out to clip a leaking aneurysm. The posterior segment shows areas of pre-retinal haemorrhages arising from the optic nerve head. The history and the fundal appearance were diagnostic of Terson's syndrome.

The haziness of the fundal view is caused by vitreous haemorrhage.
