Congenital constant exotropia

This 4 year-old boy had a large constant exotropia since birth. Cover tests showed the extropia to be alternating and the visual acuities in both eyes were normal. The extropia measured about 65 prism dioptres. He underwent a three-muscle operation involving right medial rectus resection (8mm) and lateral rectus recession (8mm) and a left lateral rectus recession (6mm). Post-operatively, he was orthotropic in the primary position.
