Congenital fibrosis syndrome

This 5 year-old boy was born with a left exotropia. Eye movement is restricted in all directions but he has no ptosis. The visual acuity is 6/6 in the right eye and CF in the left eye. The fundal examination is normal. The most likely diagnosis is congenital fibrosis syndrome involving the lateral rectus, inferior rectus and the superior rectus. However, to confirm the diagnosis a positive forced duction test is essential. The patient will undergo forced duction test under general anaesthesia and if positive surgery will be performed to centre the eye.

 

Left exotropia in the primary position.

 

The left eye fails to move in any directions.
