This 65 year-old woman had
had a left exotropia since her teenage year. She was seen in the clinic for
diabetic retinopathy screening which was unremarkable. Examination revealed
normal vision in both eyes. The exotropia was alternating. When the right
eye was covered the left eye took up fixation and remained even when the
occlusion was removed. She was likely to have intermittent exotropia
as a child which became decompensated with age. We offered her corrective
surgery to improve her appearance but she declined.
