Penetrating ocular injury

A 23 year-old mechanic sustained a left ocular penetrating injury from a piece of metal at work. He was not wearing eye protection at the time. CT scan showed a metallic foreign body in the left vitreous cavity. Slit-lamp examination showed perforating corneal injury and the presence of a dense cataract with anterior capsule breach (indicative of lenticular perforation). There was lenticulo-corneal touch when the patient was reviewed the second post-operative day and his vision was light perception. Par plana vitrectomy was performed to remove the cataract as well as the foreign body. During the foreign body removal, the retina developed tear. Silicone oil was instilled to tamponade the retinal tear and to prevent retinal detachment. The eye was left aphakic for the time being. 

 

Post corneal repair with 10/0 nylon and the presence of a traumatic cataract.

 

Lenticulo-corneal touch between the endothelium and the lenticular cortex.
