Primary acquired melanosis

Photo by Professor Chua Chung Nen

This 40 year-old woman is concerned about the increased pigmentation at the cornea near the limbus adjacent to an a patch of brown conjunctival lesion. The brown patch was first noted in her teen but shows no signs of increase in size or changes in colour. The history and appearance is suggestive of primary acquired melanosis of the conjunctiva. The increased pigmentation at the cornea is caused by migration of melanin from the lesion. She is advised to have an excisional biopsy to look for any signs of atypia but declined and chooses to have the lesion observed at 6-monthly interval.
