Primary corneal graft failure

This 75 year-old woman had poor right vision due to corneal injury sustained in her teen. A corneal graft was performed using donor cornea from Sri Lanka. At the time of surgery, the donor cornea exhibited corneal oedema. Post-operatively, the corneal graft was clear for only 48 hours. The central opacity increased rapidly and became totally opaque within 10 days. A repeated corneal graft (also from donor in Sri Lanka) was performed 2 weeks later. The second corneal graft remained clear at 7 weeks post-operative. Histology of the first corneal graft showed poor endothelium count.

 

Right corneal scarring with localized oedema.

 

Corneal graft opacity at 5 days post-grafting.

 

Complete corneal graft opacification at 10 days post-graft.

 

Repeated corneal graft at 5 days post-operative.
