This man complains of
progressive decrease in his vision in both eyes for the past 12 months. The
vision is worst on waking up in the morning and improves as the day goes on.
His vision is 6/24 in both eyes which can not be improved with pinhole. He
is known to have Fuch's dystrophy and two years ago underwent
phacoemulsification and implants. The post-operative period was marked by
prolonged corneal oedema which eventually resolved. Slit-lamp examination
shows multiple guttata, increased corneal thickness and epithelial oedema.
Application of glycerine to the right eye (after application of alcaine
eyedrop to reduce ocular discomfort) reduces the epithelial oedema and
improves his vision. He is prescribed 5% sodium chloride to be used early in
the morning to improve his vision.
