Phaco iris injury

This 68 year-old diabetic man with poorly dilated pupil underwent a left phacoemulsification and implant. During the operation, the iris was also found to be floppy with recurrent iris prolpase through the incision wounds. The inferior iris was accidentally caught by the phaco tip during sculpting causing an iatrogenic broad iridectomy. Post-operatively, the vision was 6/12 unaided.
