This 64 year-old man
underwent phacoemulsification and implant. During the stage of irrigation
and aspiration, fluid entered the stroma through a small descemet tear at
the entry site causing the descemet membrane to become separated from the
deep stroma. An air bubble was injected at the end of the surgery to aid
reattachment of the descemet membrane.

from iris prolapse during the surgery
(the patient was diabetic and had floppy iris).
