Phacoemulsification in
this patient was complicated by posterior capsule rupture which required
anterior vitrectomy. Consequently, the intraocular lens was placed in the
sulcus. Post-operatively, the refraction was more myopic than expected
because the image was focused more anteriorly. The myopia could have been
reduced by using an intraocular lens with a lower power from that calculated
preoperatively (typically by 0.5D).
